Update Child or Youth Information

Argyle Road Baptist Church collects and retains the personal information about your child for the purpose of enrolling your child/youth in our programs, to assign the student to the appropriate classes, to develop and nurture ongoing relationships with you and your child, and to inform you of program updates and upcoming opportunities at our church. This information will be kept on file for at least ten years, on advice of legal counsel.

Please note that because this form is submitted via email, it is not secure. If you would like a paper form instead, or if you wish Argyle Road Baptist Church to limit the information collected, or to view your child’s information, please contact us.

Use this form if you have already registered your child and just wish to update the child’s age, grade or other small piece of information. If you are registering another child, please use the longer form. Thank you!

However, if I cannot be reached, I authorize the ministry personnel at Argyle Road Baptist Church to sign consent for medical treatment and to authorize any physician or hospital to provide medical assessment, treatment or procedures for the participant named above.(required)

I (we), named above, undertake and agree to hold blameless the ministry staff, Argyle Road Baptist Church, its Pastors and Board from and against any loss, damage or injury suffered by the participant as a result of being part of the activities of Argyle Road Baptist Church, as well as of any medical treatment authorized by the supervising individuals representing the church. This consent and authorization is effective only when participating in and traveling to and from the events of Argyle Road Baptist Church. (required)
