Ministry Application Form – Assistants

This form is for adults and youth who wish to work as volunteer assistants in Children’s or Youth Ministries at Argyle Road Baptist Church. We believe this information is necessary to protect our children and volunteers.

Information received is confidential and is being gathered for the purposes of screening ministry personnel and placing them into ministry with children or youth. The information gathered here will be used for the purposes of supporting the ministries at Argyle Road Baptist Church.

When you submit this form, it goes to the pastor of the church for approval. It will then be stored securely in the church office and treated as confidential. The only others who will have access to it as needed are the Chair of the Deacons and the head(s) of Children’s or Youth ministries. Please note that the form is transmitted from the website to the church by email, which is not a secure form of transmission. If you would prefer a paper form, please contact the office or pick one up at the back of the sanctuary at the church.

(e.g. pornography, use of illegal substances, etc.) Answering Yes to this or any of the following 3 questions may not necessarily preclude your involvement in ministry, but we believe it is necessary to ask to provide a safe and secure environment for our children and youth. Have you ever been the subject of any disciplinary action (including discharge) or investigation by a church, religious or other organization, or by an employer?(required)

I hereby give Argyle Road Baptist Church permission to contact persons named as references, and I release both the references and the church from liability from any damage that may result. (required)

I agree to obtain and provide to the church an up-to-date police record check. (required)

I have read and understood the church’s Child and Youth Safety Manual, and I agree to abide by the child protection policies of the church. I will cooperate fully with ministry leaders and colleagues to provide a safe and wholesome experience for the children and youth we serve. I will treat all information I encounter in my role as a volunteer confidential, sharing it only with ministry leaders as appropriate. (required)

I understand that volunteer ministry at Argyle Road Baptist Church is a privilege and not a right, and that if ministry leaders develop concerns about my character, morals or conduct, the privilege of volunteer ministry may be withdrawn. If at any time I find that I cannot support the policies, practices or doctrine of Argyle Road Baptist Church, and these differences cannot be satisfactorily resolved, I will gracefully and quietly withdraw from my volunteer position.
